Job Description: We are currently seeking a dedicated and skilled Nurse Practitioners (NP) or Physician Assistants (PA) to join our team in providing exceptional healthcare services and support during home visits to patients in the Los Angeles area. As a Nurse Practitioner (NP) or Physician Assistant (PA), you will play a pivotal role in delivering high-quality medical care to seniors, ensuring their health needs are met with expertise and compassion.

Key Responsibilities

  • Perform comprehensive primary care assessments, diagnose and manage acute/chronic conditions, prescribe medications, and provide preventive care and wellness visits.
  • Develop and implement individualized treatment plans for both primary care
  • Order and interpret relevant diagnostics (labs, imaging, vascular studies) to support health management.
  • Educate patients and caregivers on self-care, disease management, and lifestyle changes.
  • Collaborate with physicians and team members to optimize outcomes.
  • Maintain accurate EMR documentation and ensure regulatory compliance.

Qualifications

  • Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license Minimum of 2 years of clinical experience. Strong diagnostic, procedural, and communication skills Experience with EMR systems and mobile documentation tools
  • Bilingual (English/Spanish) preferred to support diverse patient populations

Qualifications:

Certified PA-C or NP-BC with a valid state license. Previous experience in Geriatric care or a related field is preferred.

Strong clinical and diagnostic skills. Compassionate and patient-centered approach to healthcare. Excellent communication and interpersonal skills. Ability to work independently and as part of a healthcare team. Knowledge of senior care issues and geriatric medicine.
