About the Role

As a ClarityACP Nurse Practitioner, you will conduct structured, compassionate 30–60 minute telehealth advance care planning sessions. You’ll help patients clarify their values, goals, and care preferences, creating peace of mind and practical guidance for patients, families, and care teams. There are no direct patient or payer billing responsibilities in this role.

What You’ll Do

  • Conduct synchronous telehealth ACP conversations from a private, professional workspace.
  • Guide values clarification, surrogate decision-maker designation, and advance directive completion as appropriate.
  • Navigate state-specific requirements for advance directive and related forms (as applicable).
  • Document visits using ClarityACP’s structured templates in our EHR (CharmHealth) within 48 hours.
  • Accurately document patient location at time of service to support correct telehealth documentation requirements.
  • Use defined escalation/consultation pathways for complex cases.
  • Participate in periodic quality review and ongoing professional development.

Required Qualifications

  • Active, unrestricted Nurse Practitioner license in the state(s) where you will provide care (i.e., where the patient is physically located at the time of the telehealth visit).
  • Willingness to obtain additional state licenses as needed based on patient demand and coverage needs.
  • If required by the state where you are practicing: maintain a current, valid collaborative practice agreement with a qualifying physician and provide documentation upon request.​
  • Current BLS certification (ACLS preferred).
  • Professional liability insurance with minimum coverage of $1M per occurrence / $3M aggregate.
  • HIPAA training within the past 12 months (or willingness to complete ClarityACP training before accessing patient information).
  • Completion of ClarityACP onboarding before first patient visit (self-paced, <2 hours), including a brief 5-question check-for-understanding assessment.
  • Reliable computer with webcam/microphone and high-speed internet.
  • Quiet, private workspace appropriate for confidential telehealth visits.

Preferred Experience

  • Background in palliative care, hospice, geriatrics, oncology, or primary care.
  • Experience with advance care planning or goals-of-care conversations.
  • Telehealth experience and comfort with virtual patient interactions.
  • Bilingual Spanish/English is a plus.

Compensation & Structure

  • Rate: $70/hour.
  • Visits: Scheduled in 30-minute blocks, with flexibility for clinical complexity and required documentation.

  • Location: 100% remote.
