This position is empowered to practice and contribute throughout the care experience by leveraging our interdisciplinary approach to integrated care.This position works in tandem with the primary care team to actively manage follow up care for patients including screening reminders, lab work, prior authorizations, and other delegated tasks. </span></p><p></p><ul><li><p><span>Performs evidence-based nursing care and patient education via digital communication channels.</span></p></li><li><p><span>Provides general triage services over the phone, secure messaging, and other digital channels.</span></p></li><li><p><span>Monitors incoming messages on the Crossover Platform and collaborates with the team to assign cases to providers or respond appropriately.</span></p></li><li><p><span>Responds to incoming mental health screeners and initiates referrals as needed.</span></p></li><li><p><span>Assists the primary care providers with follow-up care for members, including screening reminders, prior authorizations, and delegated tasks.</span></p></li><li><p><span>Facilitates the population health program by identifying care gaps, participating in outreach campaigns, and maintaining proper documentation (e.g., vaccines, screeners, vital signs) to close care gaps.</span></p></li><li><p><span>Maintains proper documentation of vaccines, screeners, vital signs etc. in order to close care gaps</span></p></li><li><p><span>Works with lab vendors to obtain results and add on tests.</span></p></li><li><p><span>Completes prior authorizations for prescribed medications. </span></p></li><li><p><span>Takes initiative to create workflows and standing orders to improve efficiency and collects data to demonstrate effectiveness of interventions.</span></p></li><li><p><span>Performs other duties as assigned</span></p></li></ul><p></p><p>Required Qualifications </p><p></p><ul><li><p><span>Graduate from an accredited school of nursing.</span></p></li><li><p><span>Current Registered Nurse (RN) license in the state you are practicing.</span></p></li><li><p><span>Minimum 3 years of clinical experience.</span></p></li><li><p><span>Ability to work remotely in a quiet / private environment.</span></p></li></ul><p></p><p>Preferred Qualifications</p><p></p><ul><li><p><span>Licensed in multiple states or multistate compact license</span></p></li><li><p><span>Demonstrated strong work ethic and takes pride in a detailed oriented approach to work</span></p></li><li><p><span>Highly organized and enjoys managing projects, big and small, to completion</span></p></li><li><p><span>Creative, flexible, and navigates through obstacles</span></p></li><li><p><span>Excellent follow-through with execution skills</span></p></li><li><p><span>Able to handle ambiguity and  solving issues</span></p></li><li><p><span>Instinctively goes above and beyond for members </span></p></li></ul><ul><li><p><span>Demonstrated ability to provide excellent care and collaboration over digital communication channels including video conferencing, telephone, secure messaging, chat-based communication, and asynchronous care</span></p></li><li><p><span>Proven comfort with a variety of technology platforms including Google suite, video conferencing software, CRMs, EMRs, etc.</span></p></li></ul><h3></h3><p></p>The base pay range for this position is $50.96 to $69 per hour. Pay range may vary depending on work location, applicable knowledge, skills, and experience.
